Hospice is a sanctuary of care at the end of life. This is a journey that honours each person’s and each family’s shared vision as loved ones embark on their final journey. Each person’s journey is shaped by the care plan that starts with an understanding of who this person is and progresses to specifying end-of-life goals. We provide comfort for patients with a team of hospice nurses, and specialist practitioners from the hospital palliative care team, including doctors, social workers, therapists and trained hospice volunteers. This team decides when hospice care is appropriate and continues to coordinate care goals. Communication and compassion are at the heart of what we do: approaching the end-of-one’s life, or that of a loved one, is uncharted territory, and so communication by means of regular calls and conversations, and periodic family group meetings, is a priority for the hospice team.
